Responsibilities:
Complete comprehensive evaluations of assigned patients and develop individualized treatment plans Implement, modify, and assess therapeutic interventions in line with physician direction and patient goals Provide guidance, direction, and delegation for discipline-specific therapy services to team members Maintain timely, accurate, and compliant documentation as per legal, ethical, and facility standards Educate patients, families, and staff, fostering knowledge-sharing and best practice adherence Uphold organizational safety standards, promoting patient and employee safety in all aspects of care Participate in professional development, continuing education, and organizational performance improvement initiatives Adhere to state practice acts, maintaining licensure and professional ethics
Qualifications:
Valid Physical Therapy license (or eligibility) for Connecticut Previous experience in a hospital or outpatient setting preferred Strong clinical reasoning, communication, and documentation skills Commitment to patient-centered, safe, and ethical care Ability to work collaboratively and adapt to varying practice approaches
