Full-Time Are you a compassionate and driven Physical Therapist looking to make a meaningful impact in the lives of older adults? Join our dedicated rehabilitation team in a welcoming and supportive Skilled Nursing Facility (SNF) where your expertise is valued, and patient care is the top priority.
Position Summary:
We are seeking a licensed Physical Therapist (PT) to provide comprehensive rehabilitation services to residents with a variety of medical conditions. The PT will work closely with an interdisciplinary team to develop personalized care plans that help residents regain strength, mobility, and independence.
Key Responsibilities:
Evaluate residents and develop individualized treatment plans Deliver evidence-based physical therapy services Document progress and adjust treatment plans as needed Collaborate with physicians, nursing staff, and other rehab team members Educate residents and families on therapy goals and home exercises
Qualifications:
Current and valid Physical Therapist (PT) license in CT Graduate of an accredited Physical Therapy program Previous experience in SNF or geriatric setting preferred (new grads welcome!) Strong communication and interpersonal skills Compassionate, patient-focused, and team-oriented
