Work Environment This role is based onsite in our modern outpatient office located in Southington, a thriving community in Connecticut. Key Responsibilities Conduct thorough evaluations, including reviewing patient history and performing specific assessments to determine treatment needs. Develop and communicate tailored treatment plans that involve patients and their families in the goal-setting process. Implement treatment protocols competently based on evaluations, patient presentations, and current best practices in healthcare. Collaborate with other healthcare providers when necessary, adjusting treatment plans as needed based on observed changes in patient conditions. Ensure comprehensive discharge planning and follow-up care is executed, with all documentation completed accurately and in a timely manner, including initial evaluation notes, progress summaries, and discharge evaluations. Manage time effectively to meet productivity expectations, maintaining an organized schedule and clear communication during team meetings. Participate actively in departmental goals, comply with accreditation standards, and contribute to initiatives that remove barriers to care. Engage in ongoing professional development, seek out educational opportunities, and serve as a mentor to new clinicians and students in clinical rotations. Patient Population / Client Focus You will have the opportunity to work with a wide-ranging patient demographic, including athletes recovering from injuries, post-operative patients, and individuals managing chronic conditions. Your clinical expertise will play a crucial role in improving their mobility, alleviating pain, and restoring function, ultimately enhancing their quality of life. Qualifications A Doctorate in Physical Therapy (DPT) is required. A valid DPT license in the state of Connecticut is mandatory. Current certification in Basic Life Support (BLS) for Healthcare Providers by the American Heart Association. Commitment to ongoing learning and professional growth in the field of physical therapy. Preferred Experience While previous work experience in a clinical setting is appreciated, it is not a mandatory requirement for this role; recent graduates are encouraged to apply.
