Full-Time & Part-Time Positions Available Nurses At Home, a leading home health agency specializing in transitional care from hospital to home , is seeking a dedicated and experienced Physical Therapist to join our team. We are committed to delivering high-quality care that promotes independence, mobility, and overall well-being for our patients as they recover in the comfort of their homes.
Key Responsibilities:
Provide comprehensive physical therapy evaluations and treatments to patients in a home setting. Develop and implement individualized treatment plans tailored to patient needs and recovery goals. Collaborate closely with an interdisciplinary team, including nurses, occupational therapists, and physicians, to ensure a seamless transition from hospital to home. Educate patients and caregivers on therapeutic exercises, mobility techniques, and injury prevention strategies. Monitor patient progress, adjust treatment plans as needed, and document all care in compliance with state and agency regulations. Maintain a high standard of care , ensuring safety and effective recovery for each patient.
Qualifications:
Licensed Physical Therapist in the state of Connecticut. Home health or acute care experience preferred. Strong assessment, clinical, and communication skills. Ability to work independently while coordinating care within a multidisciplinary team. Reliable transportation to travel to patient homes within Hartford County .
