Essential Responsibilities Evaluate patients and establish individualized physical therapy treatment plans. Provide skilled therapeutic interventions to improve mobility, strength, balance, coordination, endurance, and functional independence. Assess home environments and recommend adaptive equipment to improve safety. Educate patients, families, and caregivers on treatment plans, exercises, mobility techniques, and fall prevention. Document evaluations, treatments, progress notes, and discharge summaries accurately and timely. Communicate regularly with physicians and interdisciplinary care team members regarding patient progress and changes in condition. Participate in case conferences and care coordination activities when needed. Maintain compliance with Medicare, state, and company documentation standards. Promote patient independence while improving overall quality of life. Qualifications Graduate of an accredited Physical Therapy program. Current Physical Therapist license in the State of Delaware. Minimum of six (6) months of recent experience in home health, hospital, or rehabilitation preferred. Current CPR certification. Valid driver's license, reliable transportation, and current automobile insurance. Ability to travel throughout assigned service areas. Strong clinical assessment, communication, and documentation skills. Ability to work independently while effectively managing time and priorities. Successfully pass required background and abuse registry checks. Preferred Qualifications Previous Home Health experience. Knowledge of Medicare Home Health Regulations. Experience using electronic medical record (EMR) systems.
