We are offering very competitive compensation rates. The Physical Therapist will be able to create their own schedule and will be provided with cases that are within a self-designated geographical area. This position is PRN, part-time, or full-time. Duties Assess patients' physical conditions through comprehensive evaluations. Conduct treatment visits. Collaborate effectively with healthcare professionals—including physicians, nurses, occupational therapists, and social workers—to coordinate patient care and ensure seamless treatment progress. Manage clinical documentation standards by accurately recording patient progress, treatment details, discharge planning notes, and therapy outcomes within electronic health records (EHR) systems or electronic medical records (EMR). Engage in patient communication by explaining treatment procedures clearly and empathetically to foster understanding and compliance. Experience Proven experience practicing in a physical therapy role within inpatient hospital settings, outpatient clinics, or home health environments. Familiarity with clinical documentation standards for accurate recordkeeping of physical therapy documentation and treatment plans. Prior experience working with geriatric patients in home & community care settings or outpatient rehabilitation centers is highly desirable.
