Job Description
Peoples Home Health, LLC is seeking a compassionate Physical Therapist to join our Healthcare & Medical Services team. In this field-based role, you will deliver high-quality, patient-centered physical therapy in home and community settings, supporting a safety-focused culture rooted in service and community involvement. Responsibilities include comprehensive evaluations, development of individualized treatment plans, gait and balance training, therapeutic exercise, and patient/caregiver education. You will collaborate closely with an interdisciplinary care team, maintain accurate EHR documentation, and follow strict safety and clinical protocols to optimize functional outcomes.
