Responsibilities:

Provides initial and ongoing comprehensive assessments of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments. Establishes a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary. Treats clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, water, light and electricity. Teaches, supervises, and counsels the family and client in the physical therapy program. Instructs client in care and use of wheelchairs, braces, crutches, canes and prosthetic and orthotic devices. Supervises Physical Therapy Assistants and paraprofessionals at least once monthly. Participates in case conferences, physical therapy staff meetings, agency activities and in-service programs as necessary. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, visit notes and significant change in condition in accordance with agency defined time frames.

Qualifications:

Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association. Must be licensed or registered by the applicable State without restrictions. One year of home health experience required. Must have knowledge of Medicare and Medicaid guidelines Must have a valid driver's license and reliable transportation. Must have a current CPR certification. Excellent verbal and written communication skills. Basic computer skill proficiency. Ability to use Microsoft Windows.

Background Screening Requirement:

Employment in Florida is contingent upon completion of a Level 2 background screening in accordance with Florida AHCA requirements. Applicants will be required to complete fingerprinting and background screening through the AHCA Clearinghouse.
