Job Description
Encompass Health seeks a Licensed Physical Therapist to deliver high-quality rehab care to patients recovering from major illness or injury. In this on-site role, you will evaluate patients, develop individualized treatment plans, and provide evidence-based interventions to restore mobility, function, and independence. You'll collaborate closely with an interdisciplinary team, educate patients and families, and document progress to clinical standards. Responsibilities Provide individualized physical therapy evaluations and treatment plans for post-acute and rehab patients Deliver evidence-based therapeutic interventions to restore function, mobility, and independence Collaborate with an interdisciplinary rehab team to coordinate comprehensive patient care Educate patients and families on home exercise programs, safety, and recovery expectations Document assessments, goals, and progress in accordance with clinical and regulatory standards Supervise and mentor therapy support staff as appropriate Contribute to quality improvement initiatives and best-practice clinical standards Required Skills Physical therapy evaluation and treatment planning Orthopedic and neurologic rehabilitation techniques Gait and balance training Therapeutic exercise prescription Functional mobility and transfer training Patient and family education Clinical documentation and EMR use Interdisciplinary team collaboration Pain management modalities Understanding of rehabilitation regulations and best practices
