A licensed Physical Therapist providing personalized, in-home and senior living care to older adults, utilizing a flexible schedule and incorporating telehealth when appropriate.Key Responsibilities:
Conduct evaluations, develop individualized treatment plans, deliver evidence-based therapy, and support patient independence and mobility. Document care accurately within EMR, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff to promote safety and aging in place.Qualifications & Skills:
