Job Description
Encompass Health seeks a compassionate Physical Therapist to deliver evidence-based rehabilitative care in our inpatient and home-based settings. You will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions to restore function, mobility, and independence. Collaborate closely with an interdisciplinary rehab team to ensure high-quality, patient-centered outcomes in a nurturing, dynamic environment. Responsibilities include patient and family education, accurate clinical documentation, participation in care conferences, and support of continuous quality improvement and professional development opportunities. Responsibilities Evaluate patients and perform comprehensive physical therapy assessments Develop and implement individualized, evidence-based treatment plans Provide therapeutic exercises, manual therapy, and modalities to improve function and mobility Educate patients and families on home exercise programs and safety Collaborate with physicians, nurses, and the interdisciplinary rehab team on care plans Document assessments, treatments, and progress in the electronic medical record Participate in care conferences and discharge planning Contribute to quality improvement initiatives and best-practice implementation Required Skills Patient evaluation and assessment Development of individualized PT treatment plans Therapeutic exercise prescription Manual therapy techniques Neuromuscular re-education Gait and balance training Use of rehab equipment and modalities Clinical documentation in EMR systems Interdisciplinary team collaboration Patient and family education
