Locations:
Home health visits in Manatee county (including Bradenton, Lakewood Ranch, Anna Maria Island, Ellenton, Palmetto , and surrounding areas). Visits may also be available in Sarasota and/or Hillsborough counties if desired.
Schedule :
PRN; 5-10+ home health visits per week. Required weekend/holiday on-call rotations. What you'll Do Perform comprehensive patient assessments, including OASIS assessments at required intervals Develop, initiate, and revise individualized plans of care as patient needs evolve Deliver skilled physical therapy services and treatments as outlined in the plan of care Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies Collaborate with the interdisciplinary team to ensure seamless, holistic care Why Join Empath Health as a PRN clinician?
