Job Summary We are seeking a dedicated and energetic Physical Therapist to join our healthcare team. In this vital role, you will develop and implement personalized treatment plans to promote mobility, function, and overall well-being for our diverse resident population. Your expertise will help improve quality of life through comprehensive patient care, collaboration with healthcare professionals, and adherence to clinical standards. Duties Assess residents' physical conditions through detailed evaluations and develop tailored treatment plans that address individual needs. Deliver effective physiotherapy interventions including manual therapy, trigger point therapy, and mobility exercises to support patient rehabilitation. Manage discharge planning processes to ensure seamless transitions from hospital or inpatient settings to home or community care environments. Document all patient interactions accurately in electronic health records (EHR) systems while complying with clinical documentation standards. Collaborate closely with physicians, nurses, occupational therapists, and other healthcare providers to coordinate care strategies. Provide specialized care for geriatric patients, including those in outpatient clinics, inpatient units, or home health settings. Educate patients and their families about physiotherapy techniques, injury prevention, and ongoing wellness strategies. Experience Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health environments. Demonstrated expertise in discharge planning, acute care experience, and working with geriatric populations. Strong knowledge of physiology, anatomy, orthopedics, neurology, pediatrics, and related medical terminology. Familiarity with outpatient rehabilitation center operations and outpatient clinic experience. Proficiency in electronic health records (EHR) management systems such as EMR systems; experience with inpatient and hospital settings is highly valued. Ability to develop comprehensive treatment plans based on patient assessments and clinical observations. Excellent client communication skills to foster trust and motivate patients throughout their rehabilitation journey.
