We specialize in home health and community-based therapy services, partnering with independent subcontractors who want flexibility, competitive compensation, and a supportive team behind them. We handle the administrative side so you can focus on what you love — patient care. Role Overview As a Physical Therapist subcontractor with THHN Consulting LLC, you will deliver skilled physical therapy services to patients in their homes across the West Palm Beach, FL area. Although THHN Consulting is headquartered in West Palm Beach, FL, we welcome licensed therapists from ALL Florida counties. You will work in your own community — serving patients near where you already live. No relocation required. You'll evaluate patients, develop individualized treatment plans, and provide hands-on therapy to restore mobility, reduce pain, and improve functional independence. What You'll Do Conduct comprehensive initial evaluations and reassessments in patients' homes Develop and implement individualized treatment plans based on physician referrals Provide skilled therapeutic interventions to improve mobility, strength, balance, and functional independence Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques Maintain accurate and timely clinical documentation per Medicare and payer guidelines Coordinate care with physicians, nurses, and other therapy disciplines as needed Requirements Active Florida license in Physical Therapy (or eligibility for Florida licensure) DPT or equivalent degree and active PT license in good standing Current CPR/BLS certification (BLS for Healthcare Providers) Proof of professional liability insurance Valid W-9 (we provide a blank W-9 if needed) Reliable transportation for home visits Strong documentation skills and attention to clinical detail What We Offer Per-visit compensation: Regular Visit $52 | Eval/Recert/Re-Eval/OASIS DC $70 | Start of Care (SOC) $90 Flexible scheduling — you set your availability Choose your own caseload and service area Supportive consulting team for credentialing questions Fast onboarding Work with a growing, therapist-first company
