Work Environment This position is based onsite in clients' homes throughout the Port Orange area. Working in such a dynamic environment allows for tailored and flexible care tailored to individual patient needs. Your typical day may involve traveling to multiple client locations, making each encounter unique. Team collaboration is vital, and you will have the chance to interact with healthcare professionals, families, and caregivers to ensure the best care transitions for our clients. Key Responsibilities Conduct comprehensive evaluations of patients to gather vital information, which includes interviewing patients and their families, consulting with physicians, and reviewing medical histories. Develop and implement individualized treatment plans that align with patient-specific goals and outcomes, ensuring continuity of care. Utilize evidence-based techniques and interventions tailored to each patient's needs, applying your knowledge of orthopedic conditions to enhance mobility and function. Provide in-depth education for patients and their families regarding treatment processes, preventative measures, and at-home care strategies. Regularly assess and reassess patient progress, modifying treatment plans based on observed outcomes and professional judgment. Maintain accurate patient documentation and billing records consistent with organizational standards to ensure compliance and quality of care. Diligently adhere to Florida state practice regulations and uphold the highest standards of professional ethics and safety precautions in all interventions. Effectively utilize physical therapy equipment and demonstrate its purpose to clients and families, ensuring a safe and reassuring atmosphere. Collaborate with multidisciplinary teams to enhance patient support systems, facilitating seamless communication among healthcare providers. Ensure regular and punctual attendance in line with Life and Wellness Services policy and applicable federal and state laws. Patient Population / Client Focus Your primary focus will be on orthopedic patients who are recovering from surgeries or managing chronic conditions affecting their mobility. These clients may include individuals of varying ages, from pediatric to geriatric populations, each presenting unique challenges and rehabilitation needs. You will leverage your expertise in physical therapy to deliver age-appropriate care that promotes recovery and promotes sustained health improvements. Qualifications To be successful in this role, candidates must possess the following qualifications: A Bachelor's degree or higher from an accredited program in physical therapy. A current and valid Physical Therapist license in the state of Florida; a temporary permit may be accepted during the hiring process. Certification in BLS/Healthcare Provider CPR, ensuring readiness for any emergency that may arise. Preferred Experience While not mandatory, candidates with prior experience in home care settings or those specialized in orthopedic rehabilitation will have an advantage. Familiarity with documentation practices and patient education strategies will also be beneficial to effectively carry out your responsibilities. Compensation and Benefits The salary range for this position is competitive, ranging from
USD 120,000
