The BEST work with US! The Role The home health therapist will be responsible for providing high quality and coordinated care to patients in their homes and communities. The ideal candidate will have a strong background in home health care, an understanding of state and federal regulations, and a passion for helping patients live their best lives. What You Will Do Conduct comprehensive physical therapy evaluations and create personalized treatment plans to help patients achieve their goals. Teach patients and their families how to properly perform exercises and activities to promote recovery and improve mobility. Document patient progress, including changes in condition, treatment plan, and goals, and communicate updates to other healthcare professionals. Coordinate care with other medical professionals, including physicians, nurses, social workers, and speech therapists, to ensure the most effective treatment is being delivered. Who You Will Be Graduate from an accredited physical therapy program with a valid Physical Therapy license in the state of Florida. Minimum of one year of experience in physical therapy, preferably in a home health care setting. Demonstrated ability to create effective physical therapy treatment plans that promote patient recovery.
