Family Home Care Group is seeking an experienced and compassionate Physical Therapist (PT) to perform skilled home health visits, including Start of Care (SOC) assessments , ongoing therapy visits, evaluations, reassessments, and discharge planning. If you enjoy autonomy, flexible scheduling, and making a real impact in patients' homes, we'd love to hear from you.
Position Responsibilities:
Complete comprehensive Start of Care (SOC/OASIS) assessments Develop individualized physical therapy plans of care Perform skilled PT visits, evaluations, reassessments, and discharges Improve patient mobility, strength, balance, and safety in the home Collaborate with physicians, nurses, and interdisciplinary care team Complete timely, accurate EMR documentation Educate patients and caregivers on treatment plans and fall prevention
Qualifications:
Active Florida Physical Therapist (PT) license Minimum 1 year PT experience preferred Home health experience strongly preferred
OASIS / SOC
experience preferred Reliable transportation and valid driver's license Strong clinical judgment and time management skills Current CPR certification Why Join Family Home Care Group?
