Job Description

The Physical Therapist assesses patient needs and develops individualized treatment plans designed to improve or restore physical function while promoting safe, effective, and high-quality patient care. As a
Physical Therapist, You Will:
Perform and document initial evaluations to identify patient needs, functional limitations, and appropriate therapy interventions. Develop and implement individualized treatment plans to improve mobility, strength, balance, function, and overall independence. Provide therapeutic interventions designed to alleviate pain, restore physical function, and prevent further disability. Evaluate patient progress throughout treatment and modify interventions as needed to achieve optimal outcomes. Apply infection control and universal precaution standards while providing patient care. Ensure continuity and quality of care through accurate and timely clinical documentation. Communicate effectively with patients, families, physicians, and other members of the interdisciplinary healthcare team. Educate patients and caregivers regarding treatment plans, home exercise programs, safety, and strategies to promote independence. Maintain all required documentation within company guidelines and regulatory standards. Support and promote the agency's philosophy, policies, and commitment to quality patient care. Requirements Graduate of an accredited Physical Therapy program. Current and valid Physical Therapist license in the applicable state. CPR certification. Valid driver's license. Current automobile insurance. 1 year of Home Health experience preferred. HomeCare HomeBase (HCHB) experience preferred. Physical Demands Ability to stand, walk, sit, climb, crawl, squat, or kneel as required while providing patient care. Ability to lift, carry, push, or pull up to 100 lbs. as required.
