Job Description Job Description Job Summary Our company is expanding and we are seeking a skilled Home Health Physical Therapist (PT) to join our team. In this role, you will provide individualized physical therapy services to patients in their homes, helping them improve mobility and regain independence following illness, injury, or surgery. Key Responsibilities Perform initial evaluations and develop individualized treatment plans Provide therapeutic exercises, manual therapy, and mobility training Assess patient progress and adjust care plans accordingly Educate patients and caregivers on safety and exercises Maintain timely and accurate documentation Collaborate with interdisciplinary care teams Follow home health and Medicare guidelines Qualifications Current Physical Therapist license Graduate of accredited PT program 1-2 years of experience preferred in Home Health Strong communication skills Ability to work independently Possesses and maintains current CPR certification. Valid driver's license and reliable transportation Knowledge of OASIS Experience with EMR systems such as Kinnser This role requires a Level 2 background screening in compliance with Florida AHCA regulations prior to employment.
