The ideal candidate will possess a comprehensive understanding of physiotherapy practices, with a focus on delivering exceptional patient care across diverse settings. As a Physical Therapist, you will evaluate, develop, and implement individualized care plans to promote recovery, improve mobility, and enhance quality of life for patients of all ages. Your expertise in anatomy, physiology, and medical terminology will be essential in providing effective treatment and discharge planning within inpatient, outpatient, home health, and community care environments. Duties Assess patients' physical conditions through detailed evaluations, utilizing knowledge of anatomy and physiology to identify treatment needs. Develop personalized care plans that incorporate therapeutic exercises, manual therapy techniques such as trigger point therapy, and modalities suited to each patient's condition. Implement physiotherapy interventions across various settings including hospitals, clinics, outpatient centers, and home health environments. Coordinate discharge planning by collaborating with multidisciplinary teams to ensure seamless transition from inpatient to community or home care. Monitor patient progress regularly, adjusting treatment strategies to optimize recovery outcomes. Educate patients and their families on physiotherapy techniques, injury prevention, and management of chronic conditions such as orthopedics and geriatrics. Maintain accurate documentation within electronic medical record (EMR) systems while adhering to healthcare regulations and confidentiality standards. Stay current with advancements in physiotherapy practices including trigger point therapy, orthopedic rehabilitation, and pediatric care to ensure the highest quality of patient care. Experience Proven experience working in inpatient hospital settings with a strong background in acute care environments. Demonstrated expertise in outpatient clinics or home & community care services with a focus on patient-centered treatment approaches. Familiarity with EMR systems for documentation and care coordination purposes. Extensive knowledge of medical terminology, anatomy, and physiology relevant to physiotherapy practice. Experience managing diverse patient populations including pediatrics, geriatrics, and orthopedic cases. Prior involvement in discharge planning processes within hospital or home health contexts is highly desirable. Hands-on experience with physiotherapy techniques such as trigger point therapy and orthopedic rehabilitation is preferred.
