Job Description
Wellstar Health System seeks a licensed Physical Therapist to deliver evidence-based, patient-centered care in our Healthcare & Medical Services department. In this role, you will evaluate and treat patients across diverse settings to restore function, mobility, and quality of life. Responsibilities include performing comprehensive assessments, developing individualized treatment plans, providing therapeutic interventions, and educating patients and families. You will collaborate with physicians and an interdisciplinary team, document care in the EMR, and support quality, safety, and performance improvement initiatives. Responsibilities Perform comprehensive physical therapy evaluations and develop individualized treatment plans Provide therapeutic exercises, manual therapy, and functional training to improve mobility and function Educate patients and families on home exercise programs, safety, and condition management Collaborate with physicians, nurses, and interdisciplinary team members on coordinated care plans Document evaluations, treatments, and progress notes accurately in the EMRParticipate in discharge planning and recommendations for equipment and follow-up services Contribute to quality improvement, safety, and evidence-based practice initiatives Supervise and mentor PT assistants or students as assigned Required Skills Physical therapy evaluation and treatment planning Manual therapy techniques Therapeutic exercise prescription Functional mobility and gait training Neuromuscular re-education Pain management modalities Post-surgical rehabilitation Documentation and EMR proficiency Interdisciplinary team collaboration Patient and family education
