Job Description
Licensed Physical Therapist (PT/PTA) - Home Health Heartify In Home Care is seeking a compassionate and dependable Physical Therapist (PT) or Physical Therapist Assistant (PTA) to provide home health services for a patient in Covington, GA 30014. The patient is recovering from a recent stroke (CVA) and presents with left-sided weakness, impaired balance, gait instability, and an increased risk of falls. Services have already been ordered and include physical therapy 3 visits per week for 6 weeks. The therapist will provide gait training, balance training, strengthening exercises, transfer training, therapeutic exercises, functional mobility training, patient and caregiver education, and establish a home exercise program to improve safety, mobility, and independence. Timely and accurate documentation is required. Applicants must hold an active Georgia PT or PTA license, have a valid driver's license and reliable transportation, maintain current CPR certification, and preferably have home health experience.
