Job Summary We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. In this vital role, you will assess, develop, and implement personalized treatment plans to improve patient mobility, reduce pain, and enhance overall quality of life. You will work closely with patients across various settings—including outpatient clinics, inpatient hospitals, home health environments, and community care—to deliver expert physiotherapy services that promote recovery and independence. Responsibilities Conduct thorough patient evaluations to determine functional limitations and develop tailored treatment plans based on physiology knowledge and clinical assessment. Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics, neurology interventions, and geriatrics-focused therapies to optimize patient outcomes. Collaborate effectively with healthcare professionals—including physicians, nurses, occupational therapists, and social workers—to coordinate care plans and discharge strategies. Manage electronic health records (EHR) and electronic medical records (EMR) systems accurately to document patient progress, treatment details, and clinical notes in compliance with documentation standards. Provide patient education on injury prevention, home exercise programs, and community resources to promote ongoing health management beyond clinical settings. Assist in discharge planning by evaluating patient needs for home health or outpatient rehabilitation services and ensuring seamless transitions of care. Maintain up-to-date knowledge of medical terminology, anatomy, physiology, and current physiotherapy techniques to deliver high-quality patient care across diverse populations including pediatrics, geriatrics, orthopedics, neurology, and outpatient rehabilitation. Requirements Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Proven experience practicing in a physical therapy role within inpatient hospital settings, outpatient clinics, or home health environments. Strong understanding of clinical documentation standards and proficiency managing EMR/EHR systems effectively. Demonstrated ability to develop comprehensive treatment plans tailored to individual patient needs while adhering to clinical guidelines. Experience working with geriatric patients and familiarity with discharge planning processes for community or home-based care. Knowledge of medical terminology related to orthopedics, neurology, pediatrics, and geriatrics to facilitate accurate assessments and communication. Excellent client communication skills to educate patients about their conditions and motivate adherence to therapy programs. Ability to collaborate seamlessly with multidisciplinary teams to ensure holistic patient care.
