Private PT (Physical Therapist)

Job Description We are looking for a licensed Physical Therapist to provide skilled in-home therapy services. The PT will perform evaluations, create treatment plans, and deliver interventions that support clients in improving mobility, strength, balance, and overall functional independence. Responsibilities Complete in-home physical therapy evaluations and reassessments. Develop and update individualized treatment plans based on client goals and functional needs. Provide skilled PT interventions such as gait training, balance training, therapeutic exercise, transfer training, and fall prevention. Educate clients and caregivers on exercises, mobility techniques, and safety measures. Maintain timely and accurate clinical documentation. Coordinate with the supervising nurse, agency staff, and other care team members. Follow all regulatory and agency standards for home health services. Minimum Requirements Active Georgia Physical Therapy License (PT) in good standing. Current CPR certification . Reliable transportation to complete home visits. Ability to work independently in a home setting. Strong documentation and communication skills. Minimum Qualifications At least one year of clinical experience (home health preferred but not required). Able to pass all required background checks.
