Physical Therapist - $25,000 Sign-On Bonus Candidates may live near Monticello or Cedar Rapids and/or Dubuque. - Above and Beyond Home Health Care and Hospice Agency (A Trinity Health Company) Make a Difference Every Day Join a compassionate, mission-driven team providing exceptional physical therapy services in the comfort of patients' homes. At Above and Beyond, we believe in empowering our clinicians and supporting their growth—starting with a generous $25,000 sign-on bonus . Your Role As a Physical Therapist, you'll deliver professional, evidence-based care in accordance with physician orders, state regulations, and agency policies. You'll help clients regain independence and improve their quality of life through personalized rehabilitation plans. Key Responsibilities Conduct comprehensive assessments and develop individualized treatment plans. Deliver therapeutic interventions, including manual therapy, modalities, and exercise programs. Educate clients, families, and caregivers on therapy techniques and home programs. Supervise Physical Therapist Assistants (PTAs) and Home Health Aides (as permitted). Collaborate with interdisciplinary teams and participate in care conferences. Monitor progress, adjust treatment plans, and communicate changes to physicians. Maintain accurate, timely documentation in compliance with regulations.
