Job Description
Job Overview We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. In this vital role, you will deliver comprehensive rehabilitative services to patients across various settings, including outpatient clinics, hospitals, inpatient units, and home health environments. Your expertise will help patients regain mobility, reduce pain, and improve their overall quality of life through personalized treatment plans. Responsibilities Develop and implement individualized treatment plans based on thorough assessments of patient needs and medical history Conduct patient evaluations focusing on physiology, anatomy, neurology, orthopedics, pediatrics, geriatrics, and other specialized areas Deliver therapeutic interventions including manual therapy, trigger point therapy, exercise programs, and mobility training to promote recovery and functional independence Collaborate closely with physicians, nurses, occupational therapists, and other healthcare providers to optimize patient outcomes Manage clinical documentation adhering to standards for medical terminology, physiotherapy records, and electronic health records (EHR) systems Assist with discharge planning by evaluating patient progress and coordinating follow-up care or home & community care services Educate patients and their families on treatment procedures, rehabilitation exercises, and self-care strategies to support ongoing recovery Experience Proven experience practicing in a physical therapy role within outpatient clinics or inpatient hospital settings Hands-on experience with outpatient rehabilitation centers or home health environments is highly desirable Strong background in working with diverse patient populations including pediatrics, geriatrics, neurology cases, orthopedics, and post-surgical rehabilitation Familiarity with clinical documentation standards and electronic health record (EHR) management systems such as EMR platforms Knowledge of physiotherapy techniques including trigger point therapy and treatment plan development tailored to individual needs Demonstrated ability to communicate effectively with clients and multidisciplinary teams while maintaining professionalism and compassion Prior experience in discharge planning, acute care settings, hospital environments or outpatient clinics will be considered an asset Join us in delivering exceptional patient care through innovative therapies and compassionate service.
