Hope Creek Nursing and Rehabilitation Center is seeking a dedicated and compassionate Physical Therapist to join our team! Responsibilities:
Develop and implement individualized treatment plans to achieve patient goals and outcomes Conduct thorough evaluations and assessments to identify patient needs and limitations Provide physical therapy treatments and interventions to patients, including exercise programs, manual therapy, and modalities Collaborate with other members of the interdisciplinary team to ensure comprehensive care and effective communication Monitor patient progress and adjust treatment plans as necessary Maintain accurate and detailed patient records and documentation Participate in quality improvement initiatives and stay current with best practices in physical therapy
Requirements:
Graduate degree in Physical Therapy from an accredited program Current licensure as a Physical Therapist in the state of Illinois Strong clinical skills and knowledge of physical therapy techniques and modalities Excellent communication and interpersonal skills Ability to work effectively in a fast-paced, dynamic environment
