Job Description:

The agency's physical therapist provides patient care by instructing, motivating and assisting patients in learning and improving functional activities (ex: perambulation, transfers, ambulation, daily life activities). PTs are responsible for conferring with patient family members and other health care team members to plan, modify, and coordinate patient treatment plans. We are currently looking for PTs to visit patients in the following counties:

Degree from an accredited physical therapy program• (• The American Physical Therapy Association, The Committee on Allied Health Education and Accreditation of the AMA, or the Council On Medical Education of the American Physical Therapy Association) 2 years of experience as a physical therapist Must be licensed in the

State of Illinois Responsibilities :

Reviews and evaluates physician's referral and patient medical record to ensure PT is necessary Assists physician in evaluating patient level of function to develop a plan of care (making revisions when necessary) Plan and prepares a written treatment program based on the evaluation of available patient data Prepares clinical and progress notes Participates in in-service agency programs Perform patient test, measurements, and evaluation (range-of-motion, manual muscles test, gait and functional analyses, and body parts measurements) and record and evaluate findings to aid in establishing and revising treatment plan Plan and administer prescribed physical therapy treatment for patients to restore function, alleiate pain, and prevent disability following injury, diseases, or loss of body parts Administer manual therapeutic exercises to improve or maintain muscles function, applying precise amounts of manual force and guiding patient's body part through selective patterns an d degrees of movement. Instruct, motivate, and assist patient in non-manual exercises, such as active regiments, isometric and progressive resistive and in functional activities such as using available equipment and assistive and supportive devices (such as walkers, canes, orthoses, and prosthesis Administer treatment involving application of physical agents, such as heat, light, cold, water, and electricity Evaluate, fit and adjust prosthetic and orthotic devices and recommend modifications to orthotics/prosthetist Observe, record, and report to the physician the patient's treatment response and progress and submit sixty (60) day summary Instruct other health team personnel including when appropriate, home health aides and family members in certain phases of physical therapy with which they may work with the patient Instruct patient and family in total PT program Supervision of the physical therapist assistant

Pay:

$44.39 - $53.46 per hour
