What You'll Do As a PRN Home Health Physical Therapist, you will: Evaluate patients in their homes and develop individualized treatment plans Provide one-on-one physical therapy services to improve mobility, reduce pain, restore function, and prevent disability Educate patients and caregivers on exercises, safety techniques, and home programs Monitor patient progress and adjust treatment plans as needed Complete accurate and timely clinical documentation Collaborate with physicians, nurses, and interdisciplinary healthcare teams Deliver compassionate, patient-centered care while promoting independence

Qualifications Required:

Current Illinois Physical Therapist License Previous experience in home health, rehabilitation, outpatient, acute care, or skilled nursing preferred Excellent communication and interpersonal skills Reliable transportation for travel between patient homes Ability to manage an independent schedule Commitment to providing exceptional patient care

Preferred:
