We are searching for a Part-Time or Full-Time Physical Therapist's (PT's) to cover patients in the La Grange area and the surrounding towns. Part-Time or Full-Time Physical Therapist (PT) Positions Available We are Aspire Home Healthcare, a patient centered home health agency looking for Physical Therapists (PT), for our rapidly growing organization. Ideal Physical Therapist (PT) must have strong spoken and written communication skills and capable of working with various members in the healthcare field. Candidates must advocate for their patients and be a dedicated team player. Physical Therapist (PT) considered for this position must be capable of developing and managing patient care plans and instruct both patients and families in treatment care that will promote healthy outcomes. As a physical therapist, you will create your own work schedule, provide supervision to PTA's and work closely with physicians, patients, caregivers and other ancillary support services to develop care plans to facilitate the best possible patient outcomes. The ideal candidate must have Home Health Care experience and have a dependable vehicle. Experience in orthopedics is a essential. Responsibilities and Duties
GENERAL RESPONSIBILITIES
Demonstrate working knowledge in Medicare and Medicaid regulations for home care Demonstrate understanding of organizational and therapy policies and procedures Sets appropriate priorities for managing patient caseload and schedules Consult with Administrator/Director of Therapy in meeting patient needs Demonstrate strong communication skills, both spoken and written Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed Complete patient care documentation into patient EMR in a timely manner Demonstrates ability of working independently in various communities to make home visits Demonstrates ability of providing supervision to both PTA's and documenting supervision per organizational and home care regulations Participation in in-service training and staff meetings Qualifications and Skills
QUALIFICATIONS
Physical Therapist licensed in the state of Illinois Experience in orthopedics Valid CPR Must have a dependable, insured vehicle Current Illinois Driver's License in good standing and clean record Accessible to working Smart Phone Electronic Medical Records experience a plus Able to use a laptop, internet, email, scanner/fax machine Quality Driven, patient orientated, team player
