$50-$60 per hour Job Summary We are seeking an experienced and compassionate Home Health Physical Therapist (PT) to join our growing team in the Lisle, IL area. The ideal candidate will have at least 1 year of recent home health experience and be proficient with Homecare Homebase (HCHB). This role involves providing high-quality physical therapy services to patients in their homes while collaborating with an interdisciplinary care team to improve patient outcomes and independence. Responsibilities Evaluate and treat patients in the home setting according to physician orders and care plans Develop individualized treatment plans focused on mobility, strength, balance, safety, and functional independence Complete accurate and timely documentation in Homecare Homebase (HCHB) Perform OASIS assessments as required Educate patients and caregivers on exercises, mobility techniques, safety, and home exercise programs Collaborate with nurses, physicians, case managers, and interdisciplinary healthcare teams Monitor patient progress and adjust treatment plans as needed Maintain compliance with Medicare, state, and agency regulations Qualifications Active Illinois Physical Therapist (PT) license Minimum 1 year of recent home health experience required Experience with Homecare Homebase (HCHB) required Strong knowledge of OASIS documentation Excellent communication and organizational skills Current CPR certification Valid driver's license, reliable transportation, and auto insurance Preferred Qualifications Ability to manage an independent home health caseload Strong clinical assessment and patient care skills Experience working with geriatric and post-acute populations Benefits Competitive hourly pay: $50-$60/hour Flexible scheduling Mileage reimbursement Health, dental, and vision insurance Paid time off Schedule Monday-Friday Flexible hours Weekend availability optional #WorkwolfBenefits
