Job Description

Role Overview:
Provides physical therapy evaluation and treatment for patients of all ages, adjusting care plans based on patient response and evidence. Serves as a clinical resource and role model for staff and students, ensuring safe patient transfers and promoting proper body mechanics.
Key Responsibilities:
Designs individualized care plans incorporating goals, treatment, and discharge strategies. Educates patients, families, caregivers, and healthcare team members using effective teaching methods. Performs examinations, selects appropriate assessment tools, and documents findings per policy. Evaluates impairments related to orthopedic and neurologic conditions, completing objective testing. Applies therapeutic modalities and supports patient progress with communication to referring providers. Monitors support personnel and supervises students, contributing to departmental goals.
Qualifications & Skills:
Bachelor's Degree in Physical Therapy. Licensed Physical Therapist (PT) in Illinois. Basic Life Support (BLS) certification. Strong clinical assessment, documentation, and communication skills. Knowledge of therapeutic modalities and patient safety protocols.
