Job Description

Physical Therapist (PT) -
Home Health Case Manager Location:
Chicago - South Side (servicing ZIP codes 60620, 60628, 60643 and surrounding areas)
Schedule:
Day shift with rotating weekends and holidays
Sign-On Bonus:
Eligible for up to $10,000 sign-on bonus for external candidates. Must be actively working in the same role at the time of payout. Why Join Advocate Health at Home?
