80% FTE, day shift role, variable, 7-5:30, M-F, you may be eligible for a $20,000 sign on bonus. You will work at 9th Street Clinic in Rockford. Additional components of compensation may include:

Graduate of an accredited Physical Therapy program (or foreign equivalent as determined by the State of Illinois Department of Professional Regulation). Required
Participates in educational programs on an ongoing basis and complete mandatory CEUs. Required Licenses & Certifications
Current license from the State of Illinois or in the process of acquiring licensure.
