Job Description
Job Summary We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. In this role, you will be responsible for assessing, developing, and implementing personalized treatment plans to improve patients' mobility, strength, and overall well-being. You will work closely with a multidisciplinary team to deliver high-quality patient care across various settings, including outpatient clinics, inpatient units, and home health environments. Your expertise in physiotherapy and strong communication skills will ensure patients receive effective rehabilitation services that promote recovery and independence. Responsibilities Conduct comprehensive patient assessments to determine functional limitations and develop tailored treatment plans based on clinical findings. Implement evidence-based physiotherapy interventions, including manual therapy, trigger point therapy, and therapeutic exercises to facilitate recovery. Collaborate with healthcare professionals such as physicians, nurses, and occupational therapists to coordinate patient care and discharge planning. Manage electronic health records (EHR) and electronic medical records (EMR) systems accurately to document patient progress, treatment plans, and clinical notes in compliance with documentation standards. Provide patient education on injury prevention, treatment procedures, and home exercise programs to enhance rehabilitation outcomes. Assist in discharge planning by evaluating patient progress and recommending appropriate follow-up care or community resources. Maintain up-to-date knowledge of physiology, anatomy, medical terminology, and current physiotherapy practices to deliver optimal care across diverse populations including geriatrics, pediatrics, neurology, orthopedics, and post-surgical patients. Qualifications Valid license or certification as a licensed Physical Therapist in the relevant jurisdiction. Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health settings. Strong understanding of clinical documentation standards and proficiency with EMR/EHR systems for accurate record keeping. Experience working with geriatric patients and familiarity with outpatient rehabilitation center protocols. Knowledge of physiology, anatomy, medical terminology, and specialized treatment techniques such as trigger point therapy. Ability to develop comprehensive care plans that incorporate discharge planning and patient rehabilitation strategies. Excellent client communication skills with the ability to educate patients effectively on their treatment journey.
