SIGN ON BONUS ELIGIBLE POSITION SUMMARY
: The Physical Therapist is a qualified professional who directs, supervises, evaluates and provides physical therapy services to patients as prescribed by the attending physician and under the supervision of the Regional Director of Therapy Services.
EDUCATION/EXPERIENCE
Graduate of an accredited program in Physical Therapy with a minimum of a Masters degree in PT. State of Indiana Physical Therapy License required, or eligible for state licensure within 180 days. Current CPR Certification A basic understanding of federal and state regulations regarding billing, compliance, documentation, and supervision of support staff as they relate to the practice of relevant discipline.
