Job Description
Our Health Care Facility is seeking an experienced Physical Therapist (PT) for a full-time contract position in Mishawaka, Indiana. The ideal candidate will have Skilled Nursing Facility (SNF) experience and a strong commitment to delivering high-quality, patient-centered rehabilitation services. This position offers a consistent weekday schedule with one weekend per month and some holiday coverage required. Responsibilities Perform initial and ongoing assessments of patients' functional abilities and conditions. Develop, implement, revise, and evaluate individualized plans of care based on identified patient needs and treatment goals. Involve patients and families in treatment planning and education. Provide therapeutic interventions utilizing appropriate physical therapy techniques and evidence-based practices. Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes. Maintain accurate and timely clinical documentation. Collaborate with physicians, nursing staff, rehabilitation professionals, and other members of the interdisciplinary care team. Provide direction and oversight to Physical Therapist Assistants (PTAs), students, and other rehabilitation support personnel. Ensure therapy services are provided in accordance with professional standards, facility policies, and applicable regulations. Qualifications Current Physical Therapist license or eligibility for licensure in the State of Indiana. Graduate of an accredited Physical Therapy program. 2 years of LTC or Skilled Nursing Facility (SNF) experience required Strong clinical assessment and treatment-planning skills. Excellent communication, documentation, and interpersonal skills. Ability to work effectively as part of an interdisciplinary healthcare team. Must be available to work one weekend per month and some holidays . Compensation $50.45 per hour 40 hours per week This is an excellent contract opportunity for an experienced Physical Therapist looking for a consistent weekday schedule while making a meaningful difference in patients' rehabilitation and quality of life.
