This is an exciting opportunity to be part of something new while working for a mission-driven organization dedicated to making a meaningful difference in the lives of the individuals and communities we serve. As we expand into home health services, we are looking for a clinician who is excited about the opportunity to serve patients in our growing program. In this per diem role, you will provide high-quality, patient-centered care across diverse settings while contributing to the continued growth and success of an organization focused on service, impact, and making an impact in our community. The ideal candidate will possess a strong background in occupational and/or physical therapy, with experience working with patients in a home health setting. This role offers an opportunity to contribute to treatment plan development, discharge planning, and ensuring optimal recovery and functional independence for patients.
