This is an exciting opportunity to be part of something new while working for a mission-driven organization dedicated to making a meaningful difference in the lives of the individuals and communities we serve. As we expand into home health services, we are looking for a clinician who is excited about the opportunity to serve patients in our growing program. In this per diem role, you will provide high-quality, patient-centered care across diverse settings while contributing to the continued growth and success of an organization focused on service, impact, and making an impact in our community. The ideal candidate will possess a strong background in occupational and/or physical therapy, with experience working with patients in a home health setting. This role offers an opportunity to contribute to treatment plan development, discharge planning, and ensuring optimal recovery and functional independence for patients. Most importantly, we are looking for someone who is passionate about using their skills to make an impact and is excited to grow alongside a nonprofit organization as we begin this important new chapter in home health.
Responsibilities :
The Per Diem Physical Therapist will provide skilled physical therapy services to patients in their homes, helping individuals maximize safety, mobility, independence, and functional ability. Perform comprehensive physical therapy evaluations in the home setting Develop and implement individualized plans of care Provide skilled therapeutic interventions to improve strength, balance, mobility, endurance, and functional independence Provide gait and transfer training and fall-prevention interventions Assess the patient's home environment and make appropriate safety recommendations Educate patients and caregivers regarding therapeutic programs and safe mobility Communicate effectively with physicians, nurses, occupational therapists, and other members of the interdisciplinary team Document visits and clinical findings accurately and timely Participate in care coordination and discharge planning Provide patient-centered care consistent with Medicare and home health requirements You will have the opportunity to work one-on-one with patients in their home environment and develop individualized treatment approaches based on each patient's unique needs and goals. Qualifications Current Indiana Physical Therapist license in good standing Graduate of an accredited Physical Therapy program Strong clinical assessment and documentation skills Excellent communication and interpersonal skills Ability to work independently and manage a home-based caseload Reliable transportation and ability to travel to patient homes
