Job Description
Job Summary We are seeking a dedicated and skilled Physical Therapist to join our healthcare team serving the New Haven and Fort Wayne communities. The ideal candidate will possess a comprehensive understanding of physiotherapy practices, with experience in outpatient, inpatient, and home health settings. This role involves developing personalized treatment plans, collaborating with multidisciplinary healthcare professionals, and delivering exceptional patient care to promote recovery and improve quality of life. The successful candidate will demonstrate strong clinical documentation skills, a thorough knowledge of anatomy and physiology, and a passion for rehabilitative therapy across diverse patient populations. Duties Conduct thorough assessments of patients' physical conditions, including musculoskeletal, neurological, and geriatric evaluations. Develop and implement individualized treatment plans aligned with clinical best practices and patient goals. Manage discharge planning processes to ensure seamless transition from hospital or outpatient settings to home or community care. Provide hands-on therapy such as trigger point therapy, mobility exercises, and functional training tailored to patient needs. Document patient progress accurately in electronic health records (EHR) systems while adhering to clinical documentation standards. Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to optimize patient outcomes. Educate patients and their families on treatment techniques, injury prevention, and home exercise programs. Ensure compliance with healthcare regulations and maintain high standards of safety, sanitation, and infection control within all treatment environments. Qualifications Valid licensure as a licensed Physical Therapist with relevant state credentials. Proven experience in outpatient clinics, inpatient hospital units, or home health care environments. Strong knowledge of anatomy, physiology, orthopedics, neurology, and geriatrics to inform treatment strategies. Experience working with diverse populations including pediatric, geriatric, and neurologic patients. Familiarity with clinical documentation standards and electronic health records (EHR/EMR systems). Ability to develop effective care plans based on comprehensive assessments and patient needs. Excellent communication skills for engaging with patients, families, and multidisciplinary teams effectively. Knowledge of discharge planning processes and acute care protocols is highly desirable. Pay:
