Role:
Join our dedicated team at Lincoln Park Home Health as a Physical Therapist, where you will make a meaningful impact on the lives of our patients in Abilene, KS. We are looking for compassionate professionals who are eager to provide exceptional care and support.
Responsibilities:
Conduct comprehensive assessments of patients' physical abilities and needs. Develop personalized treatment plans to enhance mobility and quality of life. Implement therapeutic exercises and techniques to promote recovery. Monitor patient progress and adjust treatment plans as necessary. Educate patients and their families on rehabilitation processes and techniques. Collaborate with a multidisciplinary team to ensure holistic care. Document patient care and maintain accurate records in compliance with regulations. Provide support and encouragement to motivate patients throughout their rehabilitation journey.
Requirements:
Valid Physical Therapy license in the state of Kansas. Bachelor's degree in Physical Therapy or related field; advanced degree preferred. Experience in home health care settings is a plus. Strong communication and interpersonal skills. Ability to work independently and as part of a team. Compassionate demeanor with a patient-centered approach. Knowledge of current physical therapy practices and techniques. Willingness to travel to patients' homes as needed.

Working Environment:
Works indoors in Agency office and in patient homes and travels to/from patient homes.
Job Relationships:
Supervised by the Therapy Program Manager and Administrator.
Exposure Risk:
High Risk Physical Requirements:
1. Ability to work for extended periods of time while standing and performing physical activity. 2. Ability to do extensive bending, lifting and standing on a regular basis.
