Responsibilities Conduct thorough patient evaluations to determine physical therapy needs, including assessments of physiology, anatomy, and medical history. Develop and implement individualized care plans tailored to each patient's specific conditions, such as orthopedics, neurology, pediatrics, geriatrics, or post-surgical rehabilitation. Collaborate closely with healthcare professionals—including physicians, nurses, and occupational therapists—to coordinate effective treatment strategies. Manage electronic health records (EHR) and electronic medical records (EMR) systems accurately to ensure compliance with clinical documentation standards. Provide patient education on therapy exercises, injury prevention, and home care routines to support ongoing recovery outside clinical settings. Perform discharge planning by preparing patients for transition to home or community care environments with appropriate follow-up instructions. Utilize specialized techniques such as trigger point therapy and other physiotherapy modalities to optimize treatment outcomes. Skills Proven experience in outpatient rehabilitation centers, inpatient hospital units, or home health environments with a focus on patient care. Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards necessary for effective treatment planning. Skilled in discharge planning and working with geriatric patients to address age-related mobility issues and chronic conditions. Ability to develop comprehensive treatment plans that incorporate physiotherapy best practices for diverse populations including pediatrics and neurology cases. Excellent client communication skills to educate patients about their care plans and motivate adherence to therapy routines. Experience working with electronic health records (EHR) systems such as EMR platforms for accurate documentation and reporting. Familiarity with outpatient clinic operations, inpatient hospital protocols, home & community care practices, and outpatient rehabilitation procedures.
