Job Description

Our dynamic team is seeking a dedicated and experienced outpatient physical therapist to join our team at Rock Creek. As an outpatient physical therapist, you will have the opportunity to work with a diverse patient population in a supportive and collaborative environment.
Responsibilities:
1. Conduct comprehensive evaluations of patients to determine their physical therapy needs and develop individualized treatment plans. 2. Provide evidence-based outpatient physical therapy services to patients with various musculoskeletal and neurological conditions. 3. Implement therapeutic exercises, manual therapy techniques, and modalities to improve patients' mobility, strength, and function. 4. Monitor patients' progress closely and adjust treatment plans as needed to ensure optimal outcomes. 5. Educate patients and their families on therapeutic exercises and home exercise programs to facilitate continued progress outside of therapy sessions. 6. Collaborate with interdisciplinary team members, including physicians, nurses, and other therapists, to coordinate patient care and achieve treatment goals. 7. Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans in accordance with facility policies and regulatory requirements.
Qualifications:
1. Graduate of an accredited physical therapy program. 2. Current state licensure as a physical therapist. 3. Minimum of 2 years of experience working as a physical therapist, preferably in an outpatient or skilled nursing facility setting. (Preferred but not required) 4. Strong clinical skills in evaluating and treating patients with orthopedic and neurological conditions. 5. Excellent communication and interpersonal skills. 6. Ability to work independently as well as collaboratively within a multidisciplinary team. 7. Commitment to providing compassionate and patient-centered care.
