Job Description

Physical Therapist Full-Time/Part-Time PRN Description The physical therapist has the responsibility to restore patient's function, alleviate pain and prevent further disabilities by planning and administering medically prescribed physical therapy.
Position Requirements Essential Job Functions/Responsibilities:
Provides physical therapy services to patients according to physician orders. This may include, but will not be limited to: Assessing and evaluating therapeutic/rehabilitative/functional status and participation in the development of the care plan. Evaluate home environment and design home exercise program. Direct physical therapy treatment and instruct patients, families and caregivers in the home exercise program. Recommends and/or provides assistive equipment for use in the patient's home. Determining priority needs for physical therapy Administers manual exercises; instructing, encouraging and assisting patients in performing physical activities, ambulatory functional activities and daily living activities. Evaluates effects of physical therapy treatments and fit of prosthetic and orthotic devices by observing, noting and evaluating patient's progress; recommending adjustments and modifications. Identifies patient and family/caregiver needs for other home health services and refers as necessary. Instructs other personnel and/or family/caregiver of areas of treatment that they may assist patient, as well as instructing them on the goals for the patient. Prepares and submits clinical and progress summaries based on the attainment of goals. Participates in discharge planning for the patient. Provides in-service education programs for home health personnel as needed. Supervises physical therapy assistants every 30 days when re-evaluation of patient is done. When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every 2 weeks.
