Job Summary:

As a Physical Therapist, you will provide skilled in-home therapy services based on individualized treatment plans, working collaboratively with physicians and interdisciplinary team members to maximize patient independence, safety, and quality of life.

What You'll Do:

Complete initial patient evaluations within 48 hours of referral and conduct reevaluations every 30 days to ensure appropriate plan of care. Develop, implement, and modify individualized treatment plans in collaboration with physicians and interdisciplinary team members. Maintain timely and accurate clinical documentation, including evaluation reports, daily notes, progress updates, and discharge summaries. Provide education to patients and families regarding safe mobility techniques, adaptive equipment, fall prevention, and home exercise programs. Communicate effectively with patients, caregivers, physicians, and agency staff to support coordinated, highquality care. Participate in case conferences, chart audits, and quality improvement initiatives to ensure regulatory compliance and positive patient outcomes.

Qualifications:

Current Kansas Physical Therapist License required. Graduate of an accredited Physical Therapy program (APTA or AMA approved). Home Health experience preferred. Minimum of two years of clinical experience preferred. Current CPR certification required. Valid driver's license, current auto insurance, and reliable transportation. Demonstrated ability to work independently while maintaining strong communication with the care team.
