This Home Health Physical Therapist opportunity is located in Ashland, Kentucky, a welcoming riverfront community nestled along the banks of the Ohio River. As a Home Health Physical Therapist in this role, you will provide one-on-one, evidence-based care to patients in their homes across Ashland and surrounding areas. You will work with a diverse adult and geriatric population, including patients recovering from orthopedic procedures, strokes, cardiac events, chronic illnesses, and deconditioning following hospital or skilled nursing stays. Your focus will be on helping patients regain functional mobility, improve safety within their home environment, and maximize independence in everyday activities. Your typical day will include traveling between patient homes within an assigned geographic area, performing comprehensive evaluations, and delivering individualized treatment sessions. You will develop and adjust plan-of-care goals, provide hands-on interventions and functional training, and design personalized home exercise programs that are realistic and meaningful to each patient's lifestyle. You will also educate patients and caregivers on safe transfers, fall prevention, use of assistive devices, energy conservation strategies, and environmental modifications to improve safety and mobility in the home. You will collaborate closely with an interdisciplinary home health team, which may include nursing, occupational therapy, speech therapy, home health aides, and medical social workers. Regular communication and case conferencing help align goals, coordinate visit frequency, and ensure that each patient's care plan is comprehensive and cohesive. Typical caseloads are structured to allow focused, quality time with each patient, while maintaining reasonable daily visit expectations and documentation requirements. Patient visits are generally scheduled during daytime hours on weekdays, with occasional flexibility needed for patient or agency needs, depending on the specific assignment. The home health setting provides a high degree of professional autonomy. You will manage your own schedule within agency guidelines, plan efficient driving routes, and balance your time between direct patient care and documentation. You will utilize an EMR system for charting assessments, visits, care plans, and communication with the broader care team. Support from clinical leadership and office staff helps ensure that you have the resources you need for scheduling, documentation, and regulatory compliance.
