Job Description

Job Overview We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will possess a comprehensive understanding of physiotherapy practices, with a passion for improving patient mobility and quality of life across diverse populations. As a Physical Therapist, you will evaluate patient needs, develop personalized care plans, and implement therapeutic interventions in various settings including inpatient hospitals, outpatient clinics, home health, and community care environments. Your expertise will contribute to restoring function, reducing pain, and promoting overall wellness through evidence-based practices. Duties Conduct thorough assessments of patients' physical conditions, including mobility, strength, and functional limitations. Develop individualized care plans that incorporate physiotherapy techniques such as trigger point therapy, orthopedics, and geriatrics interventions. Implement therapeutic exercises and modalities to improve patient outcomes in areas such as musculoskeletal injuries, post-surgical recovery, pediatrics, and chronic conditions. Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transition of care across inpatient, outpatient, home health, and community settings. Utilize electronic medical records (EMR) systems to document assessments, treatment progress, and discharge instructions accurately. Educate patients and their families on physiotherapy exercises, injury prevention strategies, and self-care techniques for ongoing wellness at home or in the community. Monitor patient progress regularly and adjust treatment plans accordingly to maximize recovery efficiency. Maintain compliance with healthcare regulations and safety standards while fostering a positive environment for patient care. Qualifications Valid licensure or registration as a licensed Physical Therapist in the relevant jurisdiction. Proven experience in acute care hospital settings, outpatient clinics, or home health environments. Strong knowledge of physiology, anatomy, medical terminology, and various physiotherapy modalities including trigger point therapy. Experience working with diverse populations such as pediatrics, geriatrics, orthopedics, and patients requiring care plans tailored to specific needs. Familiarity with EMR systems for documentation and patient management purposes. Excellent communication skills to effectively educate patients and collaborate with healthcare teams. Demonstrated ability to develop comprehensive care plans that incorporate discharge planning and community-based care strategies. Knowledge of inpatient protocols and hospital experience is highly desirable.
