Job Summary Join our dynamic healthcare team as a dedicated Physical Therapist, where your expertise will transform patient recovery journeys across diverse settings. Duties Conduct comprehensive assessments of patients' physical conditions, including mobility, strength, and functional limitations. Develop and execute tailored care plans utilizing evidence-based physiotherapy practices such as trigger point therapy and orthopedics. Deliver hands-on therapy sessions to improve patient outcomes in areas like post-surgical recovery, injury rehabilitation, and chronic pain management. Coordinate discharge planning with multidisciplinary teams to ensure seamless transitions from hospital to home or community care. Educate patients and their families on exercises, injury prevention strategies, and self-care techniques to foster long-term wellness. Maintain accurate documentation of patient progress using Electronic Medical Record (EMR) systems and adhere to medical terminology standards. Collaborate with healthcare professionals across inpatient, outpatient, home health, and clinic environments to optimize patient care and recovery pathways. Qualifications Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Demonstrated experience in acute care hospital settings, outpatient clinics, or home & community care environments. Strong knowledge of physiology, anatomy, and medical terminology essential for accurate assessments and treatment planning. Proven ability to develop effective care plans for diverse populations including pediatrics and geriatrics. Experience with EMR systems for documentation and patient record management. Familiarity with inpatient rehabilitation protocols, outpatient therapy techniques, and home health services. Skills in specialized therapies such as trigger point therapy and orthopedics are highly desirable.
