1 Description American Traveler is seeking a Physical Therapist for a Home Health assignment requiring 1+ year of recent home health experience and HCHB charting proficiency. Details Home Health setting covering a territory of up to a 60-mile radius from the office hub Caseload of 5-7 patients per day Patient population primarily geriatric Day shift schedule, 5x8-hour shifts from 8:00 AM to 5:00 PM Uses HomeCare HomeBase (HCHB) for charting No on-call requirement iPad provided for documentation; mileage reimbursed at the IRS standard rate Requirements Minimum 1 year of recent home health experience required Current BLS certification required OASIS experience required for clinicians performing Start of Care (SOC) visits HomeCare HomeBase (HCHB) charting experience required License required if applicable Proof of vehicle insurance required Must be flexible with territory coverage Additional Information Main responsibilities include providing physical therapy services to home health patients, primarily geriatric cases Scrubs required in any color; jeans not permitted First-time travelers are accepted for this position Local candidates accepted at the same travel rate Compliance items must be initiated within 24 hours of an accepted offer Any RTO requests made after an offer is accepted require approval from the on-site manager
