1+ year of acute care or related experience Pay & Benefits Pay package discussed upon application Weekly pay schedule via direct deposit Medical benefits (W-2 employment) Robust referral bonus program Paid sick time in accordance with applicable laws Licensure, certification, travel, and other reimbursements available when applicable What You'll Do Provide comprehensive physical therapy services within home health settings based on physician's plan of care Evaluate patients' physical conditions and develop individualized treatment plans Assist with therapeutic exercises, mobility training, and patient education Document patient progress and maintain accurate records using EMR (Meditech Home Care) Collaborate with interdisciplinary teams to optimize patient outcomes Support patient safety and comfort during therapy sessions Travel throughout the local 50-mile radius as neededWhat You Need Required:
Valid Louisiana state PT license BLS certification At least 1 year of acute care or home health experience Strong communication and patient care skillsPreferred:
Experience with Wound care, Heart Failure, Diabetes, IV antibiotic administration, Foley catheter, and PICC line management Knowledge of EMR systems (Meditech Home Care) Ability to work independently and flexibly within community settings About the Assignment Join a dedicated home health team providing vital therapy services to diverse patient populations.
