• , you'll contribute to our success in the following ways: + Evaluate patients and develop individualized treatment plans.

+ Provide physical therapy services in the home setting. + Monitor patient progress and adjust care plans as needed. + Collaborate with physicians, caregivers, and the interdisciplinary care team. + Complete timely and accurate clinical documentation. + Supervise therapy assistants and support staff, as appropriate.

• What is Required?
• + Graduate of an American Physical Therapy Association (APTA) approved physical therapy program + Current, unrestricted PT license valid for the state of work + Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

+ Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

_The salary range for this position is $100,000 - $115,000 annually, based on the company's good faith estimate at the time of posting._ _Actual pay will be determined based on factors such as education, experience, skills, and internal equity._ _This role is also eligible for additional compensation, which could include performance-based bonuses, overtime, on-call pay, or other incentive earnings.
